Consent to Treatment Form

For all trips and events off the Crossroads campus, we require all minors to submit a parental consent to treatment, signed by parent(s) and or legal guardian and notarized.  Click here to retrieve the form in PDF format.  

Youth Worker Application

All of those individuals who are seeking a position of employment with Crossroads Community Church, or who are seeking any position with our church involving supervision or custody of minors, or who are seeking to chaperone an overnight trip involving the supervision or custody of minors must complete the Children and Youth Worker Application and the attached Request for Criminal Records Check and Authorization.  Click here to retrieve the Application in PDF format.
